New Player to hockey Required Registration Documentation

Player has never registered in minor hockey before and lives within the Glenlake boundaries (includes players who have sibling(s) registered with Glenlake).

STEP 1 (PARENT):
  • Create a new Spordle account and link to your players.  

  • Create an HCR ID Number.  Organization must be Glenlake - do not choose Calgary or another listed association

STEP 2 (PARENT): Mandatory Hockey Canada - The Shift Forward: Evolving Hockey Culture

STEP 3 (PARENT): Proof of Player Age and Residency

E-mail the Glenlake Administrator with the following information:

  • Subject Line should read: New Hockey Registration- player’s first and last name
  • Parent Declaration Form: Please complete in full and sign the Hockey Calgary Parent Declaration Form
  • Proof of Age: Scanned copy or clear digital photo of the player’s Birth certificate or Passport
  • Proof of Residency: Please provide four (4) CURRENT proofs of residency with your name and address (kindly provide these in a document separate from proof of age). Amounts and account numbers only may be blacked out if that is your preference.
    • Acceptable Proof of Residence (POR) - PRIMARY - MINIMUM 2
      • Utility bill in your name. Bill must include service address as well as mailing address and must be current (within the last month)
      • Current GST Statement or Income Tax Assessment
      • If moving prior to the start of the season, a signed purchase/rental agreement with possession date
      • If moving to a home under construction – a signed builder agreement with possession date
      • Homeowner/Rental Insurance Policy (all pages)
      • Vehicle registration/insurance

      Acceptable Proof of Residence (POR) - Secondary

      • Cell Phone bills
      • Valid Drivers License
      • Property tax statement
      • Bank/financial statement
      • Mortgage statement
      • Student Demographic Information (available from your school

STEP 4 (ADMINISTRATOR):

    • Review and approve above documents to ensure the requirements of residency, age and Respect in Sport-Parent have been met and approval to register online
