Proof of Residence Documents

Acceptable Proof of Residence (POR) - PRIMARY - MINIMUM 2: A minimum of two of the following documents are required: a utility bill in your name that includes both the service address and mailing address and is current (issued within the last month); a current GST Statement or Income Tax Assessment; if moving prior to the start of the season, a signed purchase or rental agreement that includes the possession date; if moving to a home under construction, a signed builder agreement that includes the possession date; a Homeowner or Rental Insurance Policy (all pages); or Vehicle registration/insurance.

Acceptable Proof of Residence (POR) - Secondary: Any of the following documents are acceptable as secondary proof of residence: a cell phone bill, a valid driver's license, a property tax statement, a bank or financial institution statement, a mortgage statement, or Student Demographic Information (available from your school – this must show the player's actual residency address and not the school's address).
